Prompt users for identifier and Source of upload

When users add a new resource to Talis Elevate, we'd like users to be prompted to identify:

1. if the resource is created by them/The university for this course OR

2. If the resource is a published work, allowing them to include a known identifer for the work (ie. DOI, ISBN, etc.) or the URL they retrieve the resource from (if known) - this would be useful to see where documents are being used from subscribed/purchased online content (ie. ebooks, articles, etc.)

This would be a useful reminder to academics to consider their Copyright requirements (and reach out for support where the aren't sure), and also for Electronic Use Surveys as we could incorporate this usage data.
